Simplified Enrollment Process

We have recently modified our website so that women can begin the enrollment process with a few simple strokes of the keyboard. On the website, there is now an online form that you can submit to start the process, whereas before you needed to print out and fax or mail that document.

Don’t forget that participation is as easy as 1, 2, 3!  If you have been surgically diagnosed with endometriosis but haven’t enrolled in our study yet, now is the time!

        1.  Submit a DNA sample in the form of saliva - no blood draw is required!

        2.  Complete a questionnaire regarding family and health history

        3.  Allow us to review your medical records pertaining only to endometriosis
